WHY WE CREATED THIS MEMBERSHIP

We want to provide excellent dental care. The costs of dental insurance have been rising every year, making insurance unaffordable for many members. In order to provide affordable care for those patients without dental insurance and to encourage good dental health, Dr. John created the membership. It's for most services provided by Dentistry at Vickery Creek and Square One Dental.

You save on everything from cleanings and fillings to cosmetic procedures and crowns!

1. The membership runs on a calendar year (January to December)
2. Cost $600 for the year per person and you will have to renew it each year
3. NO deductibles
4. NO yearly maximums
5. NO claim forms
6. NO pre-authorization forms
7. NO pre-existing condition limitations
8. NO one will be denied coverage
9. NO waiting periods (start your treatment today)

Program Guidelines

• Patient's portion of bill is due the day of service.
• There will be a $50 reinstatement fee if your plan lapses.
• Cannot be used in conjunction with another dental plan or financing program such as Care Credit or Lending Club. If 3rd party financing is used, the patient will be responsible for all merchant fees.
• No refunds of premiums will be issued at any time if participant decides not to utilize dental plan.
• NON-REFUNDABLE.

Program Exclusions & Limitations

This program is a membership plan, not a dental insurance plan. It cannot be used:

• In conjunction with another dental insurance or financing program such as Care Credit.
• For Treatment, which in the sole opinion of our doctors lies out the realm of their capability.
• For referrals to specialists.
• For hospitalization or hospital charges of any kind.
• For costs of dental care which are covered under automobile medical.
• For services of injuries covered under workers' compensation.
• With any other special.

Diagnostic Exam and X-Rays

• New Patient Comprehensive Exam (new patient/initial visit)
• Periodic Exams (2 per year)
• Emergency Exams (problem-focused, 2 per year)
• 4 Bitewing X-Rays (2 times per year)
• 2 Free Periapical Films (Once per year)
• Complete Series X-Rays (FMX or Panorex), 1 every 3 years if membership is current without any lapses

Preventative

• Child Prophylaxis (2 cleanings per year)*
• Adult Prophylaxis (2 cleanings per year)*
• Flouride (2 per year, no age limit)
• Oral Cancer Screening (2 per year)
• Additional Cleanings Per Year: 10% OFF
• Dental Sealants (no age limit): 10% OFF

*Patients cannot have cleanings if diagnosed with periodontal disease.
